VITRECTOMY SURGERY

Vitrectomy is a surgical operation specifically designed to remove the vitreous gel (the transparent gel that fills the entire inner cavity of the eye).

What is a vitrectomy surgery?

Vitrectomy is the surgical technique used to treat pathologies related to the retina and the vitreous humor that, if left untreated, could cause significant vision loss.

This surgery allows the ophthalmologist to access the vitreous cavity and treat the retina, remove traction generated inside the eyeball, and eliminate opacities or inflammatory residues.

During the vitrectomy surgery, the ophthalmologist removes the vitreous humor. The vitreous humor is replaced with saline solution (physiological solution), a gas bubble, or silicone oil.

When is vitrectomy surgery necessary?  

  • Loss of transparency due to multiple diseases (e.g., inflammation or hemorrhages).
  • Retinal detachments
  • Removal of scar tissue that wrinkles or tears the retina and vitreoretinal tractions.
  • Epiretinal membranes.
  • Macular holes.
  • Trauma, severe eye injuries, or if there is a need to remove a foreign object from inside the eye as a result of an injury.

What does a vitrectomy surgery involve?

For this surgical technique, the surgeon uses a fiber optic light in one hand and a device that simultaneously cuts and aspirates (the vitreotome) in the other. This is a microincisional surgery (0.9 mm or smaller), bimanual, penetrating at the level of the sclera of the eyeball. The surgeon controls the back of the eye through constant visualization using a microscope.

During the surgery, the eye is connected to a saline infusion pump, so it is continuously filled with fluid. Meanwhile, the vitreous material, scar tissue from the retina, or any foreign objects in the eye are removed. Later, if necessary, the retina is repositioned with the help of an air bubble to maintain its correct position, and the air is replaced with gas or silicone oil to keep the retina attached to the wall of the eyeball. The gas used is absorbed over time, whereas the silicone oil must be removed in a second procedure.

Vitrectomy surgery is usually performed under local anesthesia and sedation. It is an outpatient procedure.

After Vitrectomy Surgery: Recovery and Treatment

After vitrectomy surgery, the ophthalmologist in charge of the surgery will recommend the type of recovery needed depending on each case, as well as the medications that will help relieve and heal the patient’s eye.

Generally, it is uncommon to feel pain after a vitrectomy, but various discomforts such as itching, roughness, or gritty sensations may be noticed. These will disappear over time. If an air or gas bubble has been placed, it is recommended to avoid sudden altitude changes, such as flying in an airplane, until the air or gas has been reabsorbed.

To ensure the shortest possible recovery time, it is very important that the patient follows the surgeon’s instructions properly and undergoes regular follow-up examinations.
